Immedicare LLP

Immedicare is a digitally enabled NHS service supporting care home staff 24 hours a day, 365 days a year. Formed as a joint venture in 2013, it brings together NHS clinical care and innovative technology to provide high-quality digital care. The service helps care home residents access professional clinical care through video consultations, reducing the need for referrals to other healthcare professionals, ambulance conveyance and hospital admissions. The team in the digital care hub comprises NHS nurses, pharmacists, pharmacy technicians, paramedics and healthcare support workers who provide essential additional capacity for care homes, especially out-of-hours. Immedicare supports homes with falls assessment and prevention, wound care, medicines safety management, end of life care, and nutrition and hydration. The service integrates with EMIS and SystmOne GP patient record systems and provides care homes with fully supported devices and network connectivity for high-definition video consultations. Since 2013, Immedicare has supported over 1,500 care homes and conducted over 200,000 video consultations. Almost 90% of residents remain at home following a consultation. The service aligns with the Enhanced Health in Care Homes framework and supports Integrated Care Systems.
